To request an ergonomic assessment, either fill in the referral form below, or alternatively, you can upload your own pre-filled referral form here.
Once we have received the completed form we will contact the student to book the assessment, and respond to you via email with date and time of booking.
After the assessment has taken place you will receive a copy of the report and quotation for suggested equipment within 2-3 working days.
Please ensure the uploaded file states the following;
Student contact details, students approved disability, Assessor/ assessment centre details. Any equipment to be considered during the assessment.
Additionally, if any equipment has already been recommended.
